I acknowledge that I am applying for employment with an independently owned and operated Kids' Nite Out Across America® operator (“Operator”), a separate company and employer from Kids' Nite Out Across America® and any of its affiliates or subsidiaries. I understand that each independent Operator is solely responsible for all decisions relating to employees including and without limitation hiring and termination.

I certify that the information I have provided in this application is true and complete to the best of my knowledge, and that I have made no attempt to misrepresent, conceal, or omit pertinent facts. I understand that all information provided by me in connection with my application for employment is open to investigation by Operator and that if any such information is found to be false or misleading, this may result in rejection of my employment application, withdrawal of an employment offer, immediate termination of my employment, or other disciplinary action. I further understand that the information I provide, either orally or in writing, will be relied upon by Operator in determining my employment status.

I hereby authorize Operator to request and receive reports, records and other information from any of my former employers and any other person, partnership, corporation, institution, law enforcement agency, or educational institution having knowledge of my character, general reputation, prior work record and/or education. I authorize any references I provide to grant to Operator any and all information concerning my previous employment and any pertinent information that they may have. Further, I release all parties and persons from any and all liability for any damages that may result from furnishing such information to Operator, as well as from any use or disclosure of such information by Operator or any of its agents, employees, or representatives.

As a condition of employment by Operator and in accordance with the provisions of applicable local, state and/or federal laws, I understand that if I am hired by Operator, I will be required to provide documentation verifying my identity and eligibility to work in the United States. I also understand that all offers of employment are contingent upon the receipt of satisfactory responses to reference requests and satisfactory results of background checks.

I acknowledge that if I am hired by Operator, my employment will be at will and that my employment and compensation may be terminated any time, with or without notice and with or without cause, by me or by Operator. I understand that neither this employment application nor any other document, including policies, guidelines, procedures, benefits, handbooks, or manuals, are intended to create any contractual obligation which in any way conflicts with the Operator policy that the employment relationship between Operator and each of its employees is at will. If I am hired, I agree to abide by the policies, standards, procedures, rules, and regulations of Operator.
